We used to include platform.h only when MBEDTLS_PLATFORM_C was enabled, and
to define ad hoc replacements for mbedtls_xxx functions on a case-by-case
basis when MBEDTLS_PLATFORM_C was disabled. The only reason for this
complication was to allow building individual source modules without copying
platform.h. This is not something we support or recommend anymore, so get
rid of the complication: include platform.h unconditionally.
There should be no change in behavior since just including the header should
not change the behavior of a program.
This commit replaces most occurrences of conditional inclusion of
platform.h, using the following code:
```
perl -i -0777 -pe 's!#if.*\n#include "mbedtls/platform.h"\n(#else.*\n(#define (mbedtls|MBEDTLS)_.*\n|#include <(stdarg|stddef|stdio|stdlib|string|time)\.h>\n)*)?#endif.*!#include "mbedtls/platform.h"!mg' $(git grep -l '#include "mbedtls/platform.h"')
```

After opening a file containing sensitive data, call mbedtls_setbuf() to
disable buffering. This way, we don't expose sensitive data to a memory
disclosure vulnerability in a buffer outside our control.
This commit adds a call to mbedtls_setbuf() after each call to fopen(),
except:
* In ctr_drbg.c, in load_file(), because this is only used for DH parameters
and they are not confidential data.
* In psa_its_file.c, in psa_its_remove(), because the file is only opened
to check its existence, we don't read data from it.

byte shifting opertations throughout library/ were only replaced with
the byte reading macros when an 0xff mask was being used.
The byte reading macros are now more widley used, however they have not
been used in all cases of a byte shift operation, as it detracted from
the immediate readability or otherwise did not seem appropriate.

mbedtls_dhm_get_value can be seen as either a copy function or a getter
function. Given the name and the semantics, it's more of a getter, even if
it "gets" by doing a copy. Therefore, put the context first, and the
selector next, leaving the output for last.

Add two functions mbedtls_dhm_get_len() and mbedtls_dhm_get_bitlen() to
query the length of the modulus in bytes or bits.
Remove the len field: the cost of calling mbedtls_dhm_get_len() each time
it's needed is negligible, and this improves the abstraction of the DHM
module.

Instead of generating blinding values and keys in a not-quite-uniform way
(https://github.com/ARMmbed/mbedtls/issues/4245) with copy-pasted code,
use mbedtls_mpi_random().

dhm_make_common includes a piece of code that is identical to
dhm_random_below except for returning a different error code in one
case. Call dhm_random_below instead of repeating the code.

P-1 is as bad as 1 as a blinding value. Don't accept it.
The chance that P-1 would be randomly generated is infinitesimal, so
this is not a practical issue, but it makes the code cleaner. It was
inconsistent to accept P-1 as a blinding value but not as a private key.

Unify the common parts of mbedtls_dhm_make_params and mbedtls_dhm_make_public.
No intended behavior change, except that the exact error code may
change in some corner cases which are too exotic for the existing unit
tests.

If MBEDTLS_PEM_PARSE_C is unset, the DHM selftest fails because
it uses PEM encoded test data.
This commit fixes this by providing the DER encoded form of the
test data instead in case MBEDTLS_PEM_PARSE_C is unset.
Additional changes to temporarily enable running tests:
ssl_srv.c and test_suite_ecdh use mbedtls_ecp_group_load instead of
mbedtls_ecdh_setup
test_suite_ctr_drbg uses mbedtls_ctr_drbg_update instead of
mbedtls_ctr_drbg_update_ret
This commit removes all the static occurrencies of the function
mbedtls_zeroize() in each of the individual .c modules. Instead the
function has been moved to utils.h that is included in each of the
modules.
This commit returns to using constant macros instead of global variables for the DHM group constants. Further, macros
providing the binary encoding of the primes from RFC 3526 and RFC 7919 are added. The hex-string macros are deprecated.
Although the variable ret was initialised to an error, the
MBEDTLS_MPI_CHK macro was overwriting it. Therefore it ended up being
0 whenewer the bignum computation was successfull and stayed 0
independently of the actual check.
